The general rule is that you can only upgrade an piece of equipment up to the level of the highest-level gear in its class. You can only upgrade helms up to the highest level. Weapons however can be upgraded to the level of a sword. This does not increase the weapon's output of damage and can reduce the value of its trade.
You can upgrade an item by using crests or flightstones. The amount of both depends on the item level and slot. Upgrades to swords, for example will cost more flightstones than an upgrade to an cloth. There are also discounts on the amount of flightstones and crests required to upgrade an item.
Upgrading an item fills up its diamond-shaped indicator. Each step increases the item's Tier level one-third. The maximum Tier level is Tier 3. Requirements
The WoW item upgrade system allows players to raise the level of their equipment, weapons and trinkets to their current level of item using specific Mists Of Pandaria or Warlords Of Draenor resources. If a player chooses to upgrade their item the item will be brought to the upgrader NPC, who will explain how to upgrade the item. Weapons have more strict requirements in comparison to armor.
In addition to the necessary upgrade components, many upgrade recipes also have an 'Acceptable Materials' list. This list contains the other components of an upgrade that can be used to upgrade an item. It also has an indicator that tells you whether or not the item has an assured boost. This is important to know because the value of an upgraded item can vary significantly depending on the extras it has, with well rolling gear much more valuable than the non-upgraded versions.
